Care Delivery and Patient Engagement
Providers face plenty of challenges while managing patients with multiple chronic conditions. On the other side, patients affected by chronic diseases rarely get a moment’ rest as they are not only required to manage their medications and follow care instructions but also have to continuously monitor their vitals and report the same during every hospital visit.
As per the National Health Council, chronic conditions affect 133 million people in the US approximately, which is almost 40% of the country’ population. More alarming is the fact that this number is expected to reach around 157 million by 2020 and approximately 81 million would suffer from multiple conditions.
Managing Chronic Condition
Disturbed by these staggering numbers, a leading healthcare practice approached us to design a solution that would help patients manage their health as well as allow physicians to keep a check on the risk factors for their patients.
Mobile Health Solution
To help this healthcare organization, we developed a solution that pushes notifications to take medicines and regularly record health vitals through integrated wearables, allows access to medical records, sends alerts to providers for tracking high-risk patients and mainly engages patients in their care. Regular reminders and tracking allowed physicians to get a more holistic view of their patients’ health enabling them to offer better care.
With the help of mobile devices and hospital information system integrations, we enabled the practice to seamlessly manage patients with multiple chronic conditions, achieve operational efficiency and reduce readmissions.
